Support Coordination
Dynamic Path provides support coordination that can assist you to become familiar with your NDIS Plan funding and how it can be used to help you meet your goals. Your Dynamic Path support coordinator works in partnership with you to help you design your disability supports in ways that foster your Choice and Control around your supports. Your support coordinator will work with you to negotiate with disability supports providers and help you understand how each service can work within your NDIS Plan budget. The support coordinator also ensures service agreements and service bookings are completed in a timely fashion to enable supports to be delivered on a schedule that suits you. At your request your support coordinator can also help you collect the information required by the NDIS in preparation for NDIS plan reviews. Recovery Coaching
Dynamic Path recovery coaches work in partnership with people who experience illness on building their skills and confidence to live in the community and connect to opportunities where they can make social connections, build on their life-skills and participate in activities that develop economic participation. Your Dynamic Path recovery coach will work with you to get to know you, listen to your needs and work with you to develop some short term goals about what you would like to work toward achieving on your recovery journey. Skill Development
Dynamic path workers can assist NDIS participants who may have life skills they wish to develop to help them live more independently. Dynamic Path workers can work with NDIS participants to support them to learn skills to undertake specific tasks themselves rather than having a support worker undertake these tasks for them. Dynamic Path workers provide flexible assistance to meet the specific needs of each NDIS participant by working with the participants strengths, interests and communication preferences. For example, if an NDIS participant would like to learn to use public transport, to complete their own shopping or to navigate their own local area a Dynamic path worker can work in partnership with the participant and any relevant allied health professionals to support the participant to learn these new skills.
